Basic InformationWell elder LP is an 83 year-old female born and raised in Detroit Michigan but has lived in Georgia for the past fifty-seven years Throughout her life she has been married widowed had four children and seven grandchildren Family is of utmost importance to her especially in her older age Currently she copes witha couple minor medical conditions on a daily basis including scoliosis high blood pressure a hiatal hernia and arthritis Because of the pain and energy loss caused by these conditions LP spends the majority of her time at home In terms of functioning she is able to accomplish most tasks on her own Her biggest setbacks in accomplishing activities have to do with pain and the lack of energy she experiences from time to time Frail elder MH is a 76 year-old female currently residing in metro-Atlanta She has also been married widowed had kids and grandkids While family is important to her a major portion of her time is devoted to golfing MH lives in a neighborhood with a golf course because of how much she enjoys the sport In the not-too-distant past MH began experiencing pain in her right shoulder After a consultation with the doctor it was determined that she had a tear in her rotator cuff This impacted her ability to play golf so she elected to have a right shoulder replacement She is currently in recovery and receiving home health services from an occupational therapist OT and a nurse leading her to be classified as a frail elder Her homebound status is due to the precautions she must follow as a result of her surgery she is unable to drive or leave the house without the assistance of another person The OT comes to her house in order to help her regain arm function and overall independence while the
