There remains a lack of objective semi-quantitative indexes for evaluation of single photon emission tomography SPET scan results of brain glioma patients We aimed a to validate two semi-quantitative indexes the total size index TSI and total intensity index TII of technetium-99m metaiodobenzylisonitrile 99mTc-MIBI SPET scan to discriminate high grade glioma patients versus low grade glioma patients and b to evaluate the change of viable glioma tissue by the means of the TSI and TII after surgery and after radiation treatment Thirty-two patients mean age 5518 years 20 men underwent 99mTc-MIBI SPET scan before surgery Selected 25 patients also underwent a postoperative 99mTc-MIBI SPET scan and 7 grade IV glioma patients underwent a third 99mTc-MIBI SPET scan after radiation treatment Total intensity index that corresponds to tracer uptake intensity in all segments and TSI that corresponds to the area of tracer uptake were calculated before surgery after surgery and after radiation treatment Both TII and TSI were valid in discriminating between high grade glioma and low grade glioma patients with optimal cut-off values at levels of 30 and 25 respectively Grade of glioma correlated with preoperative TSI score r076 P0001 and with preoperative TII score and r064 P0001 There was a significant decrease of TII and TSI scores after surgery in grade IV glioma patients After radiation therapy there was a significant increase of TII in grade IV glioma patients In conclusion TSI and TII were reliable in discriminating high versus low grade glioma patients and allowed for semi-quantitative evaluation of change in viable glioma tissue after surgery and after radiation treatment in grade IV glioma patients Introduction Glioma is the most common primary brain tumor in all age groups 1 2 Histological World Health Organization WHO grade remains the most reliable prognostic factor of glioma patients 3 Grade IV gliomas or glioblastomas have a devastating prognosis with only 25 of patients surviving two years with median survival time of approximately 14 months 4
