I Thomas Jones is a thirty-five year old male enrolled in the Westmorland Work Adjusment Program Tom was injured in september of 1998 while working on a roof at his previous job Martin Construction Tom sustained sever injuries to his lower back and right arm following the accident After a grueling rehabilitation process Chamberland and Gross Vocational Rehab center placed tom in his new job at Eat N Park Hear Toms new occupation is a short order cook which he found interesting during his vocational assessment Due to his back injury Tom is unable to do heavy lifting or excessive bending After a seemingly sufficient adjustment time managers find that Tom is consistantly late for work and has been praticing some unfit personal hygine methods sutied for this job Although he is not directly working with the customers other employees find it very disturbing to be in close vicinity of him Store managers have also noticed that Tom removes his hat while in the kitchen and does not tuck up his hair after removal Company policy states that no hat can be removed while in the kitchen They also mention that on numerous occations that they have caught him going to the restroom and not washing his hands before returning to work II Tom complained that due to his back injury that he was unable to do excessive lifting or bending A doctors recommendation verified that weight over forty pounds and periods exceeding one minute bending put undue strain on his back These behaviors are recognized and cannot be changed due to the functional limitation of the injury The other behaviors exhibited by Tom are all changeable The tardyness to work bad personal hygiene hair under the hat and problems with the bathroom to work transition can be corrected using one of the methods of behavior modification III Goal Get Tom to observe and abide
