I started off with preparatory phase before meeting with my patient During this phase I reviewed as much information as possible about my patient Significant data werethe patients reason for admission diagnosis health history surgical history code status diet fluid restriction fall risk vital signs and IV therapy My patient was admitted for dizziness Her diagnosis was anemia which waslow in red blood cells RBCs carry oxygen to tissues and organs and enable them to use the energy from foods Without oxygen these tissues cannot function properly Important health history that led to her anemia was her chronic kidney failure The kidneys make erythropoietin which stimulates the bone marrow to produce the proper number of red blood cells needed to carry oxygen to vital organs Since she was lacking adequate blood supplying to her tissues it was causing her to feel tire easily and weak Since she has a chronic kidney failure her BUN andcreatinine are also low Next I started with my introductory phase where I meet with my patient I introduced myself and explained the purpose and content of the interview I used nonverbal skills by using direct eye contact and sit down with my back straight before starting the interview I also observed my patients behaviors and her ability to listen actively by asking my patient if she has any difficulty hearing She replied to me that she could not hear on her right side and wanted me to be on her left side where she can hear me better I started the interview by asking what is her understanding of what brings her to the hospital She replied that she was feeling really dizzy again and needed to be hospitalized I continued to asked her how is her feelings toward her hospital stay and she replied that it was suffocating I used a clarification question by asking When you said the hospital stay was suffocating do you mean you wanted
